Your First Visit

We want your first appointment to be as smooth and productive as possible. Here's what to know before you come in.

What to Bring

  • Photo ID (driver's license or passport)
  • Insurance card(s) — medical and vision if applicable
  • List of current medications and dosages
  • Any prior medical records, imaging (MRI/CT discs or links), visual field tests, or OCT scans
  • Referral paperwork from your doctor, if applicable
  • Current glasses or contact lenses

What to Expect

  • Please arrive 15 minutes early to complete paperwork
  • A comprehensive neuro-ophthalmic evaluation typically takes 60–90 minutes
  • Your pupils may be dilated — bring sunglasses and consider having someone drive you home
  • Dr. Ghiam will review your history, perform a thorough examination, and discuss findings with you in detail
  • Additional testing (visual fields, OCT, imaging) may be ordered during or after your visit

Insurance

We accept most major medical insurance plans. Please contact our office to verify your specific plan before your visit.

Neuro-ophthalmology visits are billed through your medical insurance (not vision insurance), as these are medically necessary evaluations.
